SSWAA REGIONAL WORKSHOP WAIVER

Before registering, please be aware that SSWAA will be following current CDC suggested safety precautions at this event. Attendees will be asked/expected to comply with CDC guidelines and sign a COVID-19 waiver.

Please review and complete the following waiver. Once you complete and submit, you will automatically be redirected to the workshop registration form.

WARNING & ASSUMPTION OF RISK

There is always some level of risk when attending an event or participating in an activity. Due to the circumstances around the COVID-19 pandemic, there is an increased degree of personal risk and personal responsibility when attending a live conference.

I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected with COVID-19 by attending the SSWAA Regional Workshop and that such exposure or infection may result in personal injury, illness, disability, and death.

SAFETY PRECAUTIONS

While the School Social Work Association of America (SSWAA) is following recommended preventative measures intended to reduce the spread of COVID-19, SSWAA cannot guarantee that attendees will not be exposed or become infected. SSWAA will follow CDC safety guidelines including requiring the wearing of face masks, practicing social distancing, encouraging frequent handwashing, and offering outdoor eating options for conference meals when possible and weather permitting. SSWAA also highly encourages all attendees to get vaccinated for COVID-19 if attending the Regional Workshop in-person event. SSWAA expects all attendees to follow state/local safety guidelines and precautions in addition to respect the personal space of other attendees.

Despite precautions, I understand that the SSWAA Regional Workshop cannot guarantee that I will not be exposed or infected with COVID-19 or other communicable diseases. I agree to follow recommended safety guidelines and precautions during my attendance at the SSWAA Regional Workshop. Furthermore, I understand that these restrictions may become more strict or less strict before the event. If I am removed from the conference for not following the safety precautions, I understand I will not receive a refund.

DUTY TO SELF-MONITOR

The reduction of the spread of the COVID-19 pandemic and other communicable diseases can be impacted by diligent self-monitoring. Individuals who plan to attend the SSWAA Regional Workshop are expected to responsibly monitor their own health by watching for symptoms such as, but not limited to, fever, chills, body aches, fatigue, shortness of breath, dry cough, or difficulty breathing.

I agree to monitor my health and will not attend in-person sessions if I have symptoms or believe I have been exposed to someone with COVID-19. If I show up to the event and am symptomatic, I understand that I will be refused entry and will not be eligible for a refund.

RELEASE & WAIVER

By registering and attending SSWAA Regional Workshop, you (and/or anyone for whom you might be registering for this meeting) understand and acknowledge that there is an unknown risk of exposure to COVID-19 through exposure to contaminated objects, as well as through personal contact. Such exposure carries with it a certain degree of risk that could result in illness, disability, or death. You (and/or anyone for whom you might be registering for this meeting) assume this risk by participating in SSWAA Regional Workshop and agree to release and hold harmless SSWAA, its employees, representatives, officers, volunteers, and vendors from and against all claims of damages and liability resulting from exposure to or infection of COVID-19.
